Your privacy matters.
The collection of your health information for participating in this project is legally authorized by sections 20(b) and 27(1)(a) of the Health Information Act (Alberta). This information is collected, used, and disclosed by Primary Care Alberta (PCA) to collect pilot data on the use, feasibility, and effectiveness of a mobile digital application (i.e., the Wellnify.ai app) to support healthy behaviours for Albertans 18 years and older.
The health information that you provide will be input into Wellnify’s account management and data analytics software for the purpose of access to health behaviour tracking, educational content, and other features designed to improve people’s health behaviours in engaging, evidence-based, interactive ways.
Data on your use of the app will be used to evaluate this pilot project. Your health, demographic (age, gender), and app-use data will be separated from any personal identifiers when shared with the PCA project team for analysis. Your personal identifiers will be used only to manage your access to the app, and for sending invitations and/or reminders to voluntarily participate in evaluation activities, including surveys throughout the pilot and end-point interviews.
You may decline to participate in those evaluation activities without losing access to the app during the 12-week pilot period and with no consequence related to access to health care services. Participation is voluntary
You can stop participating at any time without affecting any care, services, programs, supports or relationships you have with partner organizations. You may stop participating at any time by discontinuing your use of the Wellnify app.
If you stop using the app, no new information will be collected from you. Information already collected prior to your withdrawal will continue to be used in project analysis and reporting.
